Common Medication Errors
According to a FDA study, 41 percent of prescription error fatalities
are due to the administration of the wrong dose of medication. Some
of the most common prescription errors include:
- Mixing up names of medications
- Disregarding allergies to medications
- Failing to check allergic reactions to certain
medications
- Failing to get all of a patient's records or information
- Failing to stay up-to-date on drug warnings
- Failing to accurately communicate drug prescriptions
(including illegible handwriting)
- Improperly labeled medication
- Misusing abbreviations, zeroes, and decimal points
in prescriptions
- Prescribing too much of a medication (wrong dosage)
- Prescribing multiple drugs that cause an adverse reaction when
taken together. (mixing the wrong drugs)
